Whenever you are doing something and don't get to the wanted/expected result for various reasons, you can create a Lesson as output of your process.
It is highly recromanded to write a short text about why things didn't work as expected. This short description is the lesson you take and share with the community.
Lessons are cited into other processes, i.e. they are attached to value streams, through what we call "solidarity mechanisms". This is recognizing that failure is part of the Design, R&D and Engineering processes and should not be punished.
Rewarding failure opens the door to abuse. Therefore, the citation of failures, as a process, should include all the participants in the process.
